Funeral Arrangement Conference Checklist

The funeral planning process begins when you visit our funeral home for the arrangement conference, but it’s helpful to come prepared. To ensure we create a meaningful tribute for your loved one, we will need some specific details. This checklist is designed to assist you in gathering the necessary information and items to make the process smoother and more personal.

  • Clothing, including undergarments, especially if you want a viewing or open casket service
  • Eyeglasses and jewelry — these can either stay with your loved one or be returned after the service
  • A recent photograph, for cosmetic purposes, if a viewing is planned
  • Military discharge papers (if applicable)
  • Social Security number
  • Life insurance policies
  • Names and contact details of pallbearers (typically 6 to 8)
  • Obituary details, including any information you’d like to include and a photo for the obituary

Death Certificate Information

We can assist in obtaining the original death certificate based on the information provided during the funeral planning conference. However, it often requires a physician’s signature to indicate the cause of death. Once signed, it is sent to the health department for registration and recording. The health department retains the original certificate, and certified copies can be requested from them. The following details are needed about your loved one:

  • Full name
  • Social Security number
  • Date and place of birth
  • Address at time of death
  • Marital status and surviving spouse’s name (if applicable)
  • Veteran’s discharge papers (DD-214) (if applicable)
  • Father’s name
  • Mother’s full name, including maiden name
  • Place of death
  • Highest level of education and occupation
